CAR 25 Forms
These are all needed forms for CAR25 membership, submission, duplication,
and permission to videotape others.
Forms may be filled out on your computer,
printed and submitted to the CAR 25 Office, or you may submit the
forms by clicking the send button on the form.

The Producer Application Form is needed to join as a member of CAR25.
A member can enter as a producer or as a non-profit organization.
Please print out and submit completed application form to
City Hall Treasurer’s office to pay membership fee.

The Permission to Videotape form is needed when producers/organizations
film the public. It is a consent for the broadcast of those individuals
in a production.
Producers, print out as many releases as needed for program
production.

The Program Duplication Form is needed when requesting the duplication
of a certain program onto DVD or VHS.

The Tape Submission Form is needed when submitting a program to air
on CAR25. The form identifies the program and the producer/organization.

Equipment Reservation Form |

In order to increase the public's knowlege of your program, we suggest
posting it on the CAR25 Bulletin Board by submitting the Video Bulletin
Board Submission Form.

The Equipment Reservation Form is needed by members when checking out
CAR25 equipment.
Producers must physically come
to the CAR25 office to complete any reservation and checkout equipment. |
